Becoming a John's Oil Customer The following information is required of all John's Oil customers: Name: Street address(including floor and/or apartment number): City: Zip code: Home telephone number: Fill pipe location while looking at front door: (example: left side,right side, front, rear) : Payment method desired: (example: cash, moneyorder. personal check*,credit/debit card**):   * - Printed name(s) on check must match the name(s) on the John's Oil account. - The driver license number of the person whose name is on the check must be provided at the time the order for oil/service is made. ** - Only MasterCard and Visa logo credit/debit cards are accepted at this time. *** - The first delivery to any address (that we have never delivered to or have not delivered to in the last 6 months) the driver is required to physically check the tank and pipes prior to delivering any oil. Delivery methods: - Will Call - You the customer call John's when you want a delivery. - Automatic Delivery - Oil is delivered to your address based on a computer program which attempts to optimize your delivery at approximately 165 gal (+/- 20 gal).
